Chicago Commons' Adult Day Services and our Home Care Services will help your senior or disabled adult continue to live in his/her home as long as possible and prevent, or delay, the need for institutional care. Our goal is to lengthen lives and build the well-being of seniors and adults with disabilities. You, as a caregiver, will get a break from the demanding responsibilities of care-giving.
At our Adult Day Care Center, we supply your senior or disabled adult with supervised care outside the home in a safe, secure "home away from home". We provide nutritious meals, individual clinical, physical and mental health care, as well as daily physical, recreational and socialization activities. Additionally, we can arrange transportation to and from your home in our wheelchair accessible busses.
Home Care Services will support your senior in his or her home. A Home Care Aide will help with personal care, cleaning/light housekeeping, medication reminders, planning and preparing meals, laundry, shopping, running errands, escorting to appointments, and “welcome back” after hospital stays. But most of all, your In-Home Care Aide will provide friendship and support for your loved-one who may be isolated through age and infirmity.
We can also arrange a combination of Adult Day Services and Home Care Services for your senior depending on your family’s needs.

Your loved one can participate in activities at Chicago Commons' Adult Day Care Center Monday through Friday from 6:30 am to 5:15 pm.
Home Care Services are provided in-home between 7:00 a.m. and 8:00 p.m. Hours of service will be determined on a case-by-case basis. Home Care Services administrative offices are open Monday through Friday from 9:00 a.m. to 5:00 p.m.
Adult Day Services — Your senior or disabled adult may enter our day care program through a state subsidy or through private pay. You can check your eligibility for a state subsidy, and what your co-payment would be, by contacting a state sanctioned Case Management office. Home Care Services — Your senior may receive home care services through a state subsidy or through private pay. Chicago Commons is now the provider of one of the largest community-based senior care programs in Chicago. The project began as a support network for individuals in Englewood and New City who were serving as informal caregivers for seniors in the neighborhood. Today, we serve over 1,000 seniors and disabled per year with Home Care and Adult Day Services.
