Senior Services

Life-Invigorating Care

As one of Chicago’s largest nonprofit providers of senior services, we support over 1,300 seniors and adults with disabilities in Illinois.

Since 1980, Chicago Commons has enabled seniors and adults with disabilities to maintain their independence and quality of life. We provide essential support to them and their caregivers through our Home Care services across the Chicago metropolitan area and our Adult Day Service Center in Bronzeville.

Adult Day Services

Enabling Seniors and Supporting Caregivers

Our Adult Day Service (ADS) program offers professional care for seniors and adults with disabilities in a community-based setting. With preventive care, health monitoring, skilled nursing services, and a Registered Nurse, we support up to 80 individuals daily, Monday through Friday.

Home Care

Personalized Care, Companionship & Support

Our Home Care services support seniors in their homes with the help of an aide. The aid visits daily or weekly to assist with activities, including:

Personal care, bath and tuck services
Light housekeeping
Medication reminders
Meal planning and preparing
Laundry, shopping, errands and escorting to appointments
“Welcome back” after hospital stays
Specialty care for Alzheimer’s, diabetes, Parkinson’s, and traumatic brain injury

Our Home Care aides also provide friendship and support for individuals who may be isolated due to age and infirmity. Services are available seven days a week, 24 hours a day and can be scheduled based on each participant’s needs.

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
