As one of Chicago’s largest nonprofit providers of senior services, we support over 1,300 seniors and adults with disabilities in Illinois.

Since 1980, Chicago Commons has enabled seniors and adults with disabilities to maintain their independence and quality of life. We provide essential support to them and their caregivers through our Home Care services across the Chicago metropolitan area and our Adult Day Service Center in Bronzeville.

Rochelle

“My mother, Sarah, has attended the Chicago Commons Adult Day Service program for over five years. Her participation in the program has positively impacted her and our family. She has a safe place to attend during the day and can socialize with her peer group. I’ve seen great improvements in her demeanor, and she is more outgoing. Her attendance within the program has prolonged her ability to continue to have a more independent lifestyle.”

Home Care

Personalized Care, Companionship & Support

Our Home Care services support seniors in their homes with the help of an aide. The aid visits daily or weekly to assist with activities, including:

  • Personal care, bath and tuck services
  • Light housekeeping
  • Medication reminders
  • Meal planning and preparing
  • Laundry, shopping, errands and escorting to appointments
  • “Welcome back” after hospital stays
  • Specialty care for Alzheimer’s, diabetes, Parkinson’s, and traumatic brain injury

Our Home Care aides also provide friendship and support for individuals who may be isolated due to age and infirmity. Services are available seven days a week, 24 hours a day and can be scheduled based on each participant’s needs.
